The true test of any insurance policy arrives at the hospital reception or TPA counter. Minor paperwork omissions often lead to unnecessary claim delays, deductions, or initial rejections. Use this standardized checklist to ensure a seamless claim settlement experience.
1. Cashless Hospitalization: Timeline & Process
A. For Planned Hospitalization (e.g., scheduled knee surgery, cataract, hernia):
- 48 to 72 Hours Prior: Visit the hospital's TPA / Insurance desk with your Policy Digital Card and patient's Government Photo ID (Aadhaar or Passport).
- Doctor's First Consultation Note: Ensure the treating doctor mentions the exact diagnosis, proposed date of admission, and expected duration of stay.
- Pre-Authorization Form: Fill Part A; the hospital doctor fills Part B. The hospital faxes/emails this to the insurer's TPA.
- Initial Approval Letter: The insurer typically issues an initial sanction letter within 1 to 3 hours.
B. For Emergency Hospitalization (e.g., sudden acute chest pain, trauma accident):
- Within 24 Hours of Admission: Notify the insurer or TPA via their toll-free helpline, mobile app, or hospital insurance desk.
- Present the physical or digital insurance card and Aadhaar card at the emergency registration desk.
2. Complete Reimbursement Claim Document Checklist
If you are hospitalized in a non-network facility or your cashless pre-authorization was converted to reimbursement, submit the following original documents within 15 to 30 days of discharge:
Mandatory Submission Dossier:
- Duly Signed Claim Form (Part A & Part B): Completely filled with no blank fields.
- Original Discharge Summary: Must state complete admission date, discharge time, medical history, clinical vitals, and surgical notes.
- Final Hospital Bill: Detailed itemized bill signed and stamped by the hospital billing department.
- Original Paid Receipts: Numbered cash/card/UPI receipts bearing official revenue stamps or bank transaction confirmations.
- Original Diagnostic & Lab Reports: X-Ray films, CT/MRI scan films and radiologist reports, pathology blood tests, and histopathology biopsy reports.
- Doctor's Prescription Sheets: Every medicine and diagnostic test claimed must have a corresponding written prescription from the treating doctor.
- Cancelled Cheque: Containing printed pre-printed name of the primary policyholder and bank IFSC code for NEFT payout.
- MLC / FIR (Only for road accidents or trauma): Medico-Legal Certificate issued by casualty and Police First Information Report copy.
Vital Rule: Always make high-resolution digital PDF scans and photocopies of EVERY document before courier dispatching or handing over original papers to the insurer or TPA. Insurers will not return original bills.
3. What to Do If Your Claim Is Unfairly Rejected
- Level 1 - Grievance Redressal Officer (GRO): Submit a formal written appeal with supporting medical certification to the insurer's GRO. The insurer must respond within 14 working days.
- Level 2 - IRDAI Bima Bharosa Portal: If the GRO fails to resolve your grievance satisfactorily, escalate via IRDAI's online portal at
bimabharosa.irdai.gov.inor toll-free at 155255. - Level 3 - Insurance Ombudsman: The Insurance Ombudsman is a quasi-judicial body created by the Government of India providing free, speedy dispute resolution. Ombudsman awards up to ₹50 Lakhs are legally binding on insurers. There are 17 Ombudsman centers across India (including Delhi, Gurugram, Mumbai, Bengaluru, and Chennai).
